All 48 test files pass (248 tests), typecheck passes via tsc --noEmit, and CLI help displays correctly via bun run dist/index.js --help

The bun:sqlite shim was needed because vitest runs under Node.js, not Bun. The pre-existing issue of missing .js extensions in compiled imports prevents node dist/index.js from working; bun run dist/index.js --help works fine.
